Registration Dates:
- Monday, April 25: 6-9 p.m., Starbucks, Route 73/Main St., Marlton
Wednesday, May 4: 6-9 p.m., Starbucks, Route 73/Main St., Marlton
Thursday, May 12: 6-9 p.m., Starbucks, Route 73/Main St., Marlton
Sunday, May 15: 10 a.m.-2 p.m., Freedom Park Gazebo, 86 Union St., Medford
New players must submit:
- MSO Registration form: Download (.doc)
- $100 cash or check to MSO on/before May 15; $125 after May 15
- Copy of driver’s license
- Copy of social security card/paystub/tax form (last four SS digits visible)
- Passport-sized (2″x2″) color photo
Forms will also be available at registration, but you may print using the link above.
Returning players must submit:
- MSO Registration form: Download (.doc)
- $100 cash/check to MSO on/before May 15; $125 after May 15.
